How to file a Federal Civil Rights complaint against the New York City Department of Health

Here is the link to file a civil rights complaint against the New York City  Department of Health with the Federal government. The Portal will ask you for a physical address of the NYC DOHM. It is not so easy to locate their address - I wonder why. So here is their address so you can fill out the form.
